Risk analysis (FMEA)

In the draft of ISO 9000:2015 companies have to perform risk analysis. The most common used risk analysis technique in industry is failure mode and effects analysis (FMEA). We are going to present a series of white papers to help companies with the implementation of FMEA. Introduction and getting started

1. Introduction to risk analysis.
In the concept version of ISO 9000:2015 risk analysis will be a requirement.  The white paper describes an introduction to risk analysis and FMEA, the history of FMEA, how to get started with FMEA and the  layout for TS16949.

2. Managing FMEA’s
What is required to make your FMEA program a success. What do you need to prepare before starting with FMEAs and what do you need to do per individual FMEA.

3. The use of classification symbols, special characteristics and RPN in FMEA
How are classification symbols and special characteristics used in FMEA. Different methods will be discussed for rating : SxOxD, OxD, SxO, Failure modes with Severity 9 and 10.  Safety and Critical failure modes.

4. Is Excel the right tool for FMEA?
To start with maybe but it soon becomes cumbersome.

5. FMEA and HACCP: A comparison 
In industry FMEA is used. In Food industry HACCP is used. What are similarities and what are differences and what would be the advantages for food industry to apply FMEA.

Check how often the FMEA has been updated and why. Check if the control plan has been updated and issued as the FMEA develops. Check the contributors to the FMEA. Over time have referential experts been brought in? Have the range of participants increased over time. The range should include operators to industry experts. Is the FMEA used to set the groups routine work?

11. FMEA as risk analysis tool for ISO 2015 (Planned January 2016)
ISO9000 will include risk analysis. FMEA is the natural tool for Process and Design. Expand it to any aspect of the organization that is a process; Purchasing, Planning , Customer feedback.

12. Using FMEA to produce machine FMEA and link this to your maintenance. (Planned March 2016)
Machine operation can be broken down into a series of operation steps. Requirements are the correct machine performance, failure modes what can go wrong with the machine, effects …what are the consequences for the machine. The corrective actions are linked to the machine maintenance program.
